Description
Adrenal gland anatomy is presented as an anterior close-up centered on the hilum, the focal region where neurovascular structures traverse the capsule. You would expect the central adrenal vein to be identified as it exits the gland, with accompanying small arterial branches (superior, middle, and inferior suprarenal arteries) entering the parenchyma from superior, medial, and inferior directions. Fine autonomic nerve bundles, predominantly preganglionic sympathetic fibers arriving via the celiac plexus and greater splanchnic nerves, converge on the gland and track alongside the vessels toward the medulla. The capsule forms the outer boundary, with the cortex lying peripheral to the medulla in the depth of field. An anterior hilum view matters because venous anatomy drives operative strategy and explains common complications: the right suprarenal vein is short and drains directly to the inferior vena cava, while the left suprarenal vein typically drains to the left renal vein (often after receiving the inferior phrenic vein). Misidentification or late control of the central vein during adrenalectomy is a common reason for bleeding and difficult exposure, and in pheochromocytoma, early ligation helps reduce catecholamine surges triggered by gland manipulation. Small arteries are numerous and variable, but the single dominant venous outflow is the landmark surgeons hunt first. Short and unforgiving.
