Description
Positioned retroperitoneally on the posterior abdominal wall, the right and left suprarenal (adrenal) glands cap the superomedial poles of the kidneys, separated from the renal parenchyma by perirenal fat and renal fascia. Superiorly, each gland abuts the diaphragm, while medially they approach the abdominal aorta on the left and the inferior vena cava on the right. Renal arteries and veins enter and exit the renal hila from a medial direction, and the ureters descend inferiorly from the renal pelvis along the psoas margin toward the pelvis. Small para-aortic and paracaval lymph nodes sit adjacent to the great vessels. Deep anatomy, clearly organized. A posterior perspective matters because the adrenals are approached and displaced in many retroperitoneal operations, and their short, asymmetric venous drainage creates predictable hazards. The right suprarenal vein drains directly into the inferior vena cava, while the left suprarenal vein typically empties into the left renal vein, anatomy that shapes hemorrhage risk and exposure during adrenalectomy and nephrectomy. This view also supports teaching the anatomic basis of adrenal hemorrhage, pheochromocytoma workup, and patterns of nodal spread near the aorta and vena cava.
