Posterior abdominal anatomy frames the paired adrenal (suprarenal) glands as flattened, cap-like organs seated on the superomedial poles of the kidneys, immediately inferior to the diaphragm. Medially, each gland relates to the vertebral column and crura, while the psoas major and quadratus lumborum form a muscular bed posterolaterally. Running in the same retroperitoneal field, the lumbar plexus branches course within and along psoas major, while segmental vessels and the major abdominal trunks occupy a more medial plane against the lumbar vertebrae. Orientation is explicit. A posterior perspective is the cleanest way to teach the adrenal glands as retroperitoneal structures that sit deep to the posterior abdominal wall musculature, and it clarifies why posterior flank approaches to the kidney can place the adrenal at risk when mobilization extends superiorly. The view also supports discussion of adrenalectomy planning, where the short, fragile right suprarenal vein draining to the inferior vena cava and the longer left suprarenal vein draining to the left renal vein influence exposure and vascular control, even when the operative corridor begins posteriorly.
