Description
Running longitudinally along the posterolateral lumbar spine, the iliocostalis lumborum is highlighted as the lateral column of the erector spinae, positioned lateral to longissimus and separated from it by a shallow fascial interval. Inferiorly, its fibers arise from the broad common erector spinae tendon anchored to the posterior iliac crest, dorsal sacrum, and sacroiliac ligaments, then ascend to attach onto the angles of the lower ribs. Superficial landmarks include trapezius superiorly and latissimus dorsi inferolaterally, with the posterior deltoid and triceps brachii framing the shoulder and arm contours. Posterior exposure makes the iliocostalis lumborum easy to teach because you can relate muscle bulk directly to the iliac crest and rib angles, two palpable guides when orienting students and when planning posterior lumbar approaches. Low back pain workups often distinguish paraspinal strain from facet-mediated pain, and this muscle is a frequent generator of tender myofascial trigger points just lateral to the spinous processes and above the iliac crest. Its relationship to the thoracolumbar fascia also matters in posterior spinal surgery and during erector spinae plane blocks, where injectate spreads deep to the erector spinae group and can track cranio-caudally along fascial planes.
