Arising from the internal pudendal artery within the ischioanal fossa, the inferior rectal artery courses medially toward the anal canal and divides into small branches that wrap around the inferior aspect of the external anal sphincter. Lateral to the anorectal junction, the vessel runs in a fat-filled corridor between the pelvic diaphragm superiorly and the skin of the perineum inferiorly, with accompanying veins that mirror its path. Portions of the sigmoid colon and rectum sit superior and posterior to this field, while perirectal lymphatics and nodal chains track alongside the mesenteric and pelvic vessels.
