Description
Arising most often from the anterior division of the internal iliac artery, the inferior vesical artery courses anteroinferiorly along the lateral pelvic wall toward the bladder base and vesical neck in the male pelvis. From an inferior perspective, its trajectory can be read as it crosses the pelvic floor deep to the levator ani fascia, then turns medially toward the posteroinferior bladder, sending branches toward the prostate and seminal vesicle. Small vesical branches run superiorly on the bladder wall, while prostatic and deferential twigs pass posterior to the pubic symphysis and anterior to the rectum. Orientation from below makes the relationship to the urogenital diaphragm and periprostatic tissues unambiguous. A short, teachable vessel. This angle matters in operative anatomy because the inferior vesical artery often contributes to the prostatic capsular blood supply and can be a source of bleeding during transurethral resection of the prostate or open/robotic simple prostatectomy, where the vesicoprostatic junction is mobilized. Pelvic oncologic surgery also brings it into play, as lymph node dissection along the internal iliac system and dissection near the seminal vesicles can place these branches at risk. Variants are common, including replacement by a branch of the middle rectal artery or a shared trunk with prostatic arteries, so a clean inferior view helps clinicians anticipate what they may encounter.
