Description
Running obliquely superomedial from the iliac crest and thoracolumbar fascia toward the linea alba, the internal oblique is presented as a deep layer of the anterolateral abdominal wall in a female figure seen from the anterior aspect. Its muscular belly sits deep to the external oblique and superficial to the transversus abdominis, with fibers that become an aponeurosis contributing to the anterior rectus sheath above the arcuate line. Inferiorly, the lowest fibers sweep toward the inguinal ligament and form part of the conjoint tendon with transversus abdominis near the pubic crest, framing the superficial planes over the rectus abdominis. Orientation matters here because the internal oblique is the muscle layer you split, not cut, in classic muscle-splitting approaches to the right iliac fossa (for example, appendicectomy via a McBurney-type incision) and in open inguinal hernia repair when defining the posterior wall of the inguinal canal. It is also the key landmark for regional anesthesia of the anterior abdominal wall: a transversus abdominis plane (TAP) block targets the neurovascular plane between internal oblique and transversus abdominis where the thoracoabdominal nerves (T7 to T11), subcostal nerve, and L1 branches (iliohypogastric and ilioinguinal) travel. Know this plane. Misplacement explains failed blocks and unintended femoral nerve spread. Use this illustration in gross anatomy and kinesiology teaching to compare fiber direction across external oblique, internal oblique, and transversus abdominis, and to map how the rectus sheath is assembled in the anterior abdominal wall.
