Description

Anterolateral abdominal wall musculature is presented with the internal oblique muscle as the primary layer of interest, lying deep to the external oblique and superficial to the transversus abdominis (highlighted in blue). Internal oblique fibers run superomedially from the iliac crest and thoracolumbar fascia toward the inferior ribs and the linea alba, while the deeper transversus abdominis courses more horizontally, wrapping the trunk like a cinching band. Medially, both muscles transition into broad aponeuroses that contribute to the rectus sheath around rectus abdominis, converging toward the linea alba; inferiorly, their fibers and aponeuroses relate to the inguinal ligament and the lateral border of rectus at the semilunar line. Layering is the point. For teaching and clinical anatomy, the relationship between internal oblique and transversus abdominis matters because it defines the neurovascular plane of the abdominal wall, where the thoracoabdominal nerves (T7 to T11), subcostal nerve (T12), and often the iliohypogastric and ilioinguinal nerves course between these two muscles. That plane underpins the transversus abdominis plane (TAP) block used for postoperative analgesia after cesarean delivery and other lower abdominal procedures, and it also explains why splitting muscle layers along fiber direction reduces denervation during anterolateral approaches. The internal oblique also forms, with transversus abdominis, the conjoint tendon medially, a landmark in inguinal hernia repair.
