Description
Posteriorly oriented, the kidney is opened to expose renal parenchyma, with the cortex forming the peripheral band and the medulla arranged into pyramids whose papillae project toward the renal sinus. Between pyramids, renal columns extend from cortex to sinus, and the minor calyces coalesce into major calyces and the renal pelvis as they converge medially at the hilum. The renal artery enters posteromedially and branches toward the segmental distribution, while renal veins course anterior to the artery toward the inferior vena cava. Yellow autonomic fibers track along the renal vessels, and green lymphatic channels accompany the same pedicles. A posterior perspective earns its place when you want to teach or plan approaches that come through the flank rather than the peritoneal cavity, because the relationship of the renal hilum to posterior abdominal wall structures governs safe access. Clear separation of cortex and medulla also supports discussion of corticomedullary perfusion and the pathway of urine from collecting ducts at the papilla into the calyceal system. Calyceal orientation and the renal pelvis configuration influence percutaneous nephrolithotomy, nephrostomy tube placement, and where an obstructing calculus at the ureteropelvic junction will back pressure into the kidney.
