Description
Running from the cervical vertebrae (C2 to C7) through the thoracic and lumbar levels to the lumbosacral junction, the intervertebral discs are highlighted in blue as fibrocartilaginous symphyses interposed between adjacent vertebral bodies. From an anterior perspective, each disc sits inferior to its corresponding vertebral body endplate, with the cervical discs smaller and the lumbar discs thicker in the cranio-caudal direction. The sacrum and coccyx appear as fused segments inferiorly, where true intervertebral discs are absent except at L5 to S1. Disc spaces align in the midline, forming a segmented column that emphasizes the transition in size from cervical to lumbar regions. Anterior disc anatomy matters because it corresponds to the anterior longitudinal ligament and the ventral annulus fibrosus, structures directly encountered during anterior cervical discectomy and fusion (ACDF) and anterior lumbar interbody fusion (ALIF). Disc height loss at C5 to C6, C6 to C7, or L4 to L5 and L5 to S1 translates into foraminal narrowing and radicular symptoms, even when the herniation itself is posterolateral. Degenerative disc disease often announces itself first as asymmetric narrowing and osteophyte formation along the anterolateral vertebral body margins. A clean view of the disc spaces helps you teach those patterns.
