Thoracic vertebral laminae form paired plates of the vertebral arch that converge in the posterior midline to meet at the base of the spinous process. From a superior perspective, the vertebral canal sits centrally, bordered anterolaterally by the pedicles and posterolaterally by the laminae as they slope toward the midline. Lateral to the lamina-pedicle junction, the superior articular processes and their facets are positioned posterior to the vertebral body, helping define the posterior wall of the thoracic spinal canal. Surgical corridors through the posterior thoracic spine depend on an exact understanding of laminar thickness, the interlaminar space, and the relationship of lamina to the spinous process and facet joints, because these landmarks guide thoracic laminectomy, laminotomy, and posterior decompression for canal stenosis or ossification of the ligamentum flavum.
