Running superficially along the anterolateral hip and thigh, the lateral femoral cutaneous nerve (lateral cutaneous nerve of the thigh) is traced from the lumbar plexus on the lateral pelvic wall toward the anterior superior iliac spine. From a lateral perspective in the adult male, its course is shown passing deep to the inguinal ligament near the ASIS, then emerging into the subcutaneous tissue to supply sensation over the lateral thigh. Iliacus and psoas major lie medial and deep to its pelvic segment, while sartorius and tensor fasciae latae form key anterior and lateral neighbors as the nerve enters the thigh. A small-caliber structure. Easy to miss. Clinically, this nerve matters because it is a pure sensory nerve that is frequently compressed where it crosses the inguinal ligament, producing meralgia paresthetica with burning dysesthesia over the lateral thigh without motor weakness. The lateral view clarifies the entrapment-prone relationship to the ASIS and the inguinal ligament, a relationship that guides landmark-based nerve blocks and helps surgeons avoid iatrogenic injury during anterior approaches to the pelvis and hip (including iliac crest bone graft harvest and hip arthroplasty exposures). Variations in its branching and its exact distance from the ASIS explain why symptoms can persist after apparently uncomplicated operations.
