Description
Arcing around the lateral malleolus, the lateral malleolar artery is presented in lateral profile as it courses inferior to the distal fibula and tracks along the anterolateral ankle toward the dorsum of the foot. Proximal to the ankle, the anterior tibial artery continues distally to become the dorsalis pedis artery, with small malleolar branches crossing the joint capsule and contributing to the periarticular arterial network. Superficial veins accompany the arterial pathways, running in the subcutaneous plane lateral to the Achilles tendon and over the lateral hindfoot, while deeper vessels remain closer to the interosseous membrane and extensor compartment. Bony landmarks anchor the scene, with the talus and calcaneus positioned inferior and medial to the fibular tip. Spatially, this lateral malleolar vascular arcade matters because it is where small-caliber vessels are exposed to trauma and surgical dissection around the distal fibula. Lateral malleolar fractures, syndesmotic fixation, and lateral ankle arthroscopy portals can disrupt these branches, contributing to wound-healing problems or a painful postoperative hematoma in the anterolateral gutter. The anastomoses between perforating fibular (peroneal) branches, anterior tibial contributions, and dorsalis pedis collaterals also explain why distal perfusion may persist despite a proximal stenosis, yet fail in severe peripheral arterial disease or diabetes.
