Across the posterior trunk, the paired latissimus dorsi muscles form broad, triangular sheets spanning the lower thoracic region into the lumbar back, with their medial fibers approaching the thoracolumbar fascia along the spinous processes and their lateral fibers sweeping superolaterally toward the posterior axillary fold. Superiorly, the inferior angles of the scapulae sit on the upper part of this muscular field, while the vertebral column remains centered in the midline from cervical to sacral levels. The iliac crests and posterior pelvis provide the inferior bony boundary, and the humeri frame the lateral margins of the back. Clinically, this posterior perspective is the one surgeons and anatomists rely on to understand how the latissimus dorsi couples trunk motion to glenohumeral mechanics, drawing the humerus into extension, adduction, and internal rotation via its insertion on the intertubercular sulcus.
