Arising from the superior aspect of the left hepatic lobe, the left hepatic vein courses posterosuperiorly to join the inferior vena cava along the liver’s posterior surface. From this posterior perspective, the vein’s larger trunk and its segmental tributaries are visible within the parenchyma, converging toward the hepatocaval junction. The inferior vena cava sits posterior and slightly medial to the left hepatic vein, with the venous branches radiating laterally through the left lobe substance. Blue venous channels contrast against the semi-transparent hepatic tissue. Posterior visualization of the left hepatic vein matters when you need to understand hepatic outflow anatomy rather than inflow. During left hepatectomy, living-donor left lateral sectionectomy, or laparoscopic resection near segments II and III, the course and entry point of the left hepatic vein into the vena cava define a key plane of dissection and a common source of troublesome venous bleeding if the vein is avulsed or inadequately controlled. Variant drainage patterns, including accessory left hepatic veins or a common trunk with the middle hepatic vein, change where surgeons can safely divide parenchyma and how interventionalists plan hepatic venography or balloon-occlusion studies. Small errors here have big consequences.
