Description
Posteriorly, the distal humerus with its olecranon fossa sits superior to the proximal ulna, whose olecranon process forms the bony tip of the elbow, while the proximal radius lies lateral to the ulnar trochlear notch. A fibrous capsular thickening spans the back of the joint, blending with the posterior capsule and reinforcing the humeroulnar articulation as it courses between the medial and lateral epicondyles. Medially, the ulnar (medial) collateral ligament complex is implied by its posterior band running toward the ulna, and laterally the radial (lateral) collateral ligament complex tracks toward the annular ligament around the radial head. Spatial orientation is clear: olecranon and posterior capsule are superficial, the trochlea and capitulum remain anterior and largely hidden. Posterior elbow ligament anatomy matters because extension places the posterior capsule and the posterior band of the ulnar collateral ligament under tension while the olecranon engages the olecranon fossa, a relationship that explains common patterns of posterior elbow stiffness after immobilization. This view also supports understanding posterolateral rotatory instability, where injury to the lateral ulnar collateral ligament permits pathologic external rotation and subluxation of the ulna relative to the humerus, often assessed with the pivot-shift maneuver and addressed with lateral ligament reconstruction.
