Description
Posterior to the knee joint, the popliteal fossa forms a diamond-shaped window where the popliteal artery and popliteal vein course deep to the superficial fascia and between the femoral condyles superiorly and the proximal tibia and fibula inferiorly. Genicular branches sweep medially and laterally around the femoral epicondyles toward the capsule and articular margins, with accompanying veins draining back into the popliteal vein. Proximally, the femur frames the superior boundary, while the tibial plateau and fibular head sit inferior and slightly lateral, providing fixed bony landmarks for the neurovascular bundle. Red arterial channels and blue venous channels are differentiated for quick orientation. Clinically, this posterior perspective matters because the popliteal artery is tethered at the adductor hiatus proximally and the tendinous arch of soleus distally, so knee dislocation and hyperextension injuries can produce intimal tears and thrombosis even when distal pulses initially seem acceptable. This region also concentrates the genicular anastomosis that can maintain distal perfusion when the superficial femoral or popliteal artery is stenosed, and it is the corridor targeted during posterior approaches for popliteal aneurysm repair or vascular bypass.
