Posterior anatomy of the male knee is rendered with the popliteal fossa opened to the vascular plane, centering the popliteal artery and popliteal vein as direct continuations of the femoral vessels after they traverse the adductor hiatus. Proximal to the joint line, the vessels lie deep to the superficial fascia and small saphenous vein, then course inferiorly against the posterior femur and capsule, with the artery typically deeper and slightly medial to the vein. Around the condylar region, genicular branches arc medially and laterally to participate in the periarticular genicular anastomosis, while distal continuation toward the trifurcation region is suggested by tapering caliber below the fossa. Transparent skeletal context keeps the distal femur and proximal tibia as fixed landmarks for orientation. Popliteal vasculature is a frequent point of failure and a surgical hazard. Posterior tibial plateau fractures and knee dislocations can injure or thrombose the popliteal artery because the vessel is tethered at the adductor hiatus proximally and the soleal arch distally, limiting excursion during trauma; missed injury threatens limb viability. For operative planning, the posterior view clarifies why the artery is at risk during posterior capsular work and why vascular surgeons favor exposure through a posterior approach in select popliteal aneurysm repairs, while also explaining the collateral routes that maintain distal perfusion when the main trunk is compromised.
