Description
Posteriorly, the occipital bone and posterior calvaria frame a dense scalp vasculature that spreads superiorly over the vertex and laterally toward the mastoid region. Along the midline of the neck, the cervical vertebrae provide bony reference for the paired vertebral arteries ascending through the transverse foramina, while more superficial branches track in the subcutaneous tissues of the nuchal region. Occipital arteries and veins course from the superior nuchal line toward the posterior scalp, and inferiorly the vascular pathways continue toward the root of the neck where the subclavian system would be encountered deep to the clavicle and superior to the scapulae. Landmarks are clear. Posterior head and neck vascular anatomy matters when bleeding control and flap perfusion depend on predictable scalp and nuchal vessels. The occipital artery, a branch of the external carotid, is a common pedicle for posterior scalp reconstruction and can be injured in lacerations over the superior nuchal line, where brisk hemorrhage is typical because the galea and dense connective tissue limit vessel retraction. Deep to the posterior triangle, the vertebral artery’s relationship to C1 to C2 is central to understanding rotational vertebral artery occlusion (Bow Hunter syndrome) and to avoiding iatrogenic injury during upper cervical instrumentation. Neurosurgery and otolaryngology texts can pair this view with posterior approaches to the craniovertebral junction, while anatomy lab manuals can use it to orient students to external carotid versus vertebrobasilar territories on the back of the head and neck. In clinical education, it supports teaching points for scalp laceration repair, occipital nerve blocks given near the occipital artery pulse, and posterior cervical exposure planning.
