Description
Visible from a superior cranial angle, the scalp and upper neck vasculature is organized around the external carotid system and its superficial venous partners, laid over the calvaria and upper cervical vertebrae. The superficial temporal artery and vein ascend anterior to the auricle and course superiorly in the temporal region, while the occipital artery and occipital vein track posteriorly toward the superior nuchal line. Medially, deeper cervical conduits are implied by the course of the common carotid artery transitioning to the internal carotid artery and the internal jugular vein within the carotid sheath, positioned lateral to the cervical vertebral column and deep to the mandibular angle. Orientation matters. From above, you can read the anastomotic pattern between facial, temporal, and occipital circulations as it spreads across the galea aponeurotica, and you can appreciate why scalp lacerations bleed briskly, the dense connective tissue tethers vessels open and prevents retraction. This perspective also supports procedural planning: the superficial temporal artery is a palpable landmark for temporalis region incisions and is the most common donor in extracranial to intracranial bypass, and its tortuosity helps it tolerate jaw motion. Venous outflow pathways toward the external and internal jugular systems are clear and clinically relevant in head and neck infection spread and in evaluating traumatic or iatrogenic vascular injury.
