Description
Medial to the knee joint, the distal femur meets the proximal tibia at the medial femoral condyle and medial tibial condyle, with the patella remaining anterior to the articulation. Along the medial joint line, branches of the popliteal artery give rise to the genicular network, with the superior and inferior medial genicular arteries coursing anteriorly around the femoral and tibial margins while companion veins track alongside in blue. Small periarticular vessels run within the capsule and adjacent connective tissue, crossing superficial to the medial meniscus and near the tibial attachment of the medial collateral ligament. Collateral circulation around the knee matters because arterial inflow is routinely challenged, either intentionally during vascular reconstruction or unintentionally after trauma and dislocation. Occlusion or injury of the popliteal artery can be limb threatening, and the genicular anastomoses and recurrent branches from the anterior tibial system provide the main bypass routes that keep distal perfusion viable. This medial perspective also clarifies why medial approaches, from MCL repair to pes anserinus procedures, demand respect for the superficial venous channels and the periarticular arterial arcades that can bleed briskly.
