Description
Posteriorly, the rectum descends from the distal sigmoid colon to the anal canal, with the mesorectum tapering inferiorly toward the pelvic floor. Arterial inflow is suggested by the superior rectal artery continuing from the inferior mesenteric artery, with anastomotic contributions from the middle rectal arteries laterally and the inferior rectal arteries approaching the anal canal from the internal pudendal system. Venous channels mirror this arrangement, with a submucosal rectal venous plexus draining superiorly toward the superior rectal vein and portal system, and inferiorly and laterally toward the middle and inferior rectal veins, then into the internal iliac and internal pudendal veins. Small perirectal lymph nodes and autonomic nerve fibers are positioned posterolaterally along the rectal wall. A dense plexus. This posterior perspective helps clarify a clinically messy area: the portosystemic communications at the anorectal junction. Dilatation of the internal rectal venous plexus underlies internal hemorrhoids, while external hemorrhoids involve the perianal veins below the pectinate line, a distinction that also tracks with pain patterns and lymphatic drainage. The route of inferior rectal vessels and nerves along the ischioanal fossa matters during drainage of ischioanal abscesses and during lateral internal sphincterotomy, where injury risks bleeding and sensory disturbance.
