Description
Running from the distal fibula (lateral malleolus) to the talus and calcaneus, the lateral collateral ligaments of the ankle are the dominant bands in this side view, with the anterior talofibular ligament positioned anteroinferior to the malleolus, the calcaneofibular ligament descending more vertically to the lateral calcaneus, and the posterior talofibular ligament spanning posteriorly toward the talus. Deep to the malleolus, the fibularis (peroneus) longus and brevis tendons course posterior to anterior within the retromalleolar groove, held by the superior and inferior fibular retinacula. More distally, the lateral midfoot stabilizers are suggested by stout fibers around the subtalar and calcaneocuboid regions, including the dorsal calcaneocuboid ligament and the calcaneofibular relationship across the subtalar joint. Bone landmarks include the fibula laterally, the tibia more medial and largely obscured, and the talus sitting superior to the calcaneus. Lateral ankle anatomy is where inversion injuries declare themselves. The anterior talofibular ligament is most often torn in plantarflexion inversion sprains, while the calcaneofibular ligament becomes involved as instability progresses and can correlate with subtalar laxity on stress testing. Clear depiction of the retromalleolar tendons also supports clinical teaching on peroneal tendon subluxation after superior fibular retinaculum disruption, a frequent source of persistent lateral ankle pain in athletes.
