Description
Seen from the lateral aspect, the distal femur lies superior to the proximal tibia, with the fibular head positioned posterolateral to the tibial plateau as a key landmark. Along the lateral joint line, the fibular (lateral) collateral ligament runs from the lateral femoral epicondyle to the head of the fibula, while the iliotibial tract courses longitudinally on the anterolateral thigh and crosses the lateral femoral condyle toward Gerdy’s tubercle on the anterolateral tibia. Posteriorly, the tendon of biceps femoris approaches the fibular head, and the posterolateral capsular thickening (often represented as the arcuate ligament complex) blends with the joint capsule and fascia. Clear layering is the point. Lateral ligament anatomy matters because varus stress and external rotation loads are restrained by a coordinated set of structures rather than a single band, and this view makes those relationships readable at a glance. Clinically, injuries to the fibular collateral ligament and posterolateral corner often accompany anterior cruciate ligament tears, and missed posterolateral instability is a known cause of ACL graft failure and persistent rotatory laxity. Orthopedic exam maneuvers such as the varus stress test, dial test, and posterolateral drawer map directly onto the structures arranged in this lateral corridor. Use this illustration when teaching knee joint stability in gross anatomy, sports medicine, and orthopedic surgery blocks, or when diagramming the lateral approach and repair/reconstruction planning around the fibular head and common peroneal (fibular) nerve corridor.
