Description
Running vertically in the anterior abdominal wall, the linea alba forms a midline tendinous raphe where the right and left rectus sheaths decussate, extending from the xiphoid process and costal cartilages superiorly toward the pubic crest and pubic symphysis inferiorly. From a lateral perspective, its superficial position becomes clear relative to the rectus abdominis, with the aponeuroses of the external oblique, internal oblique, and transversus abdominis converging toward the midline. Bony landmarks of the male trunk, including the rib cage, lumbar vertebral column, and pelvis, frame the course and tapering of the raphe. Midline anatomy, clarified. Clinically, this view supports accurate planning of midline abdominal incisions, where the linea alba is often chosen to reduce muscle splitting and to provide a relatively avascular approach, but it also highlights why closure technique matters. Weakening and widening of this raphe under raised intra-abdominal pressure contributes to diastasis recti, and defects adjacent to or through the midline can present as epigastric hernias. For trocar placement in laparoscopy, appreciating the linea alba’s relationship to the rectus sheath and umbilical region helps avoid traversing rectus muscle and its epigastric vessels. Use it in gross anatomy and surgical anatomy teaching to orient students to abdominal wall layers and the midline aponeurotic architecture, or in general surgery and plastic surgery materials addressing laparotomy, umbilical hernia repair, and abdominoplasty planning.
