Description
Running along the anterior midline of the abdominal wall, the linea alba forms a fibrous raphe where the right and left rectus sheaths interweave from the xiphoid process superiorly to the pubic symphysis inferiorly. On either side, the aponeuroses of external oblique, internal oblique, and transversus abdominis course medially to contribute to the rectus sheath, meeting at the linea alba medial to the paired rectus abdominis muscles. Superficial landmarks such as the umbilicus typically interrupt the midline, while the costal margin and anterior superior iliac spine help orient the superior and inferior extents in an adult male abdomen. Understanding the linea alba matters because it is both a surgical corridor and a mechanical weak point. Midline laparotomy and many trocar placements exploit this relatively avascular plane to reduce muscle splitting and segmental nerve injury, but the same collagenous seam is the site of diastasis recti and can widen under chronically increased intra-abdominal pressure. Herniation through the midline, including umbilical and epigastric hernias, is better explained when you can visualize how the broad abdominal aponeuroses decussate and thin as they approach the raphe.
