Description
Anteriorly oriented on a standing male child in anatomical position, the liver fills the right hypochondrium and extends medially into the epigastrium, tucked immediately inferior to the diaphragm and deep to the lower costal cartilages. The right lobe dominates the upper right abdomen, while the left lobe tapers across the midline toward the left upper quadrant. Along the visceral (inferior) surface, the gallbladder sits in its fossa between the right lobe and quadrate region, positioned just lateral to the midline and inferior to the porta hepatis. A cutaway through the right anterior thoracoabdominal wall exposes hepatic parenchyma in situ. Pediatric liver anatomy matters because the organ is proportionally larger and more inferiorly positioned than in adults, so its inferior margin can be palpable below the right costal margin even without pathology. That distinction comes up daily when teaching abdominal examination and when interpreting right upper quadrant pain, jaundice, or hepatomegaly in children, where differential diagnoses range from viral hepatitis to biliary obstruction and metabolic storage diseases. The relationship to the gallbladder also helps explain why pediatric cholecystitis is less common than adult disease yet gallbladder hydrops and hemolysis-associated pigment stones still present with localized tenderness and ultrasound findings.
