Posteriorly, the pancreas lies as an elongated, lobulated gland spanning the epigastrium into the left upper quadrant, with its head nested in the C loop of the duodenum and its tail extending toward the splenic hilum. A semi-transparent dorsal torso provides context, placing the gland anterior to the vertebral column and major retroperitoneal vessels, and posterior to the stomach and proximal small bowel. The body of the pancreas crosses obliquely, sitting superior to the transverse colon and mesocolon. Retroperitoneal by default. Orientation matters because posterior relationships govern both symptoms and surgical access. Pancreatic head carcinoma can obstruct the distal common bile duct at the hepatopancreatic ampulla, producing painless jaundice and a distended gallbladder (Courvoisier sign), while inflammation in acute pancreatitis often tracks posteriorly into the lesser sac and retroperitoneal spaces, sometimes causing back pain and fluid collections. A posterior view also clarifies why the tail, closely related to the splenic vessels and splenic hilum, is at risk during distal pancreatectomy and splenectomy, and why the pancreatic neck overlies the formation of the portal vein behind it.
