Description
Running superiorly along the posterolateral neck, the longissimus capitis is rendered as a straplike column of the erector spinae complex, positioned deep and posterior to the sternocleidomastoid and lateral to the semispinalis capitis. Its fibers ascend from the transverse processes of the mid to upper cervical vertebrae toward the mastoid region, lying just posterior to the cervical articular pillars and facet joints. Superiorly, the muscle approaches the temporal bone near the mastoid process, with the occiput and mandible providing cranial landmarks in lateral profile. Inferiorly, the cervical spine continues into the upper thorax, where ribs, clavicle, and scapula establish the shoulder girdle context. Lateral presentation matters because longissimus capitis is often confused with more superficial rotators and extensors, yet it is a frequent generator of posterior neck pain and ipsilateral occipital referral when overloaded. Palpation and injection planning depend on appreciating its depth relative to the sternocleidomastoid and splenius capitis, and its relationship to the transverse processes and cervical facet joints implicated in whiplash associated disorders and cervicogenic headache patterns. Dorsal rami of cervical spinal nerves supply this muscle, so segmental irritation can present as localized paraspinal tenderness rather than radicular symptoms. Small muscle, big consequences.
