Description
Arising from the transverse processes of the mid to lower cervical vertebrae (typically C4 to C7), the longissimus capitis forms a thick longitudinal strap that ascends superolaterally to its insertion on the mastoid process of the temporal bone, just posterior to the sternocleidomastoid attachment. In lateral profile, its fibers sit deep to the splenius capitis and posterior to the levator scapulae, while remaining continuous inferiorly with the longissimus cervicis component of the erector spinae mass. The cervical vertebral arches and the cranial base provide fixed bony landmarks that clarify the muscle’s oblique line of pull during extension and ipsilateral rotation. Functionally, longissimus capitis is one of the posterior cervical extensors that becomes clinically relevant in whiplash-associated disorders, where eccentric loading can provoke myofascial pain and limit active cervical extension. Its proximity to the mastoid process also makes it an anatomic reference when teaching surface palpation around the posterolateral neck and when interpreting lateral neck imaging, since bulk changes in the posterior paraspinal compartment may reflect denervation, chronic postural overload, or compensatory hypertrophy.
