Description
Arising segmentally from the posterior aspect of the abdominal aorta, the lumbar arteries course laterally across the anterolateral surfaces of the lumbar vertebral bodies, then swing posterior to the psoas major toward the quadratus lumborum and the layered musculature of the posterior abdominal wall. From a posterior perspective, their dorsal branches can be followed passing medially toward the vertebral arch and spinal canal, while more lateral branches track inferior to the 12th rib and above the iliac crest. Posterior elements of the lumbar spine, sacrum, coccyx, and the iliac bones frame the vascular network, with accompanying lumbar veins returning medially toward the inferior vena cava. Small-caliber arterioles and venous tributaries interlace with the paraspinal soft tissues. This view matters because the lumbar arteries sit in the working corridor for retroperitoneal and lateral transpsoas approaches to the lumbar spine, where segmental vessel injury can produce brisk hemorrhage and obscured planes. Their spinal branches also supply the vertebral bodies and contribute to radiculomedullary circulation, a point that becomes clinically concrete in aortic surgery or endovascular repair when segmental arteries are sacrificed and spinal cord ischemia is a concern. Nerve and vessel run close. Identifying their relationship to the transverse processes, psoas major, and quadratus lumborum is often the difference between a controlled exposure and a complication. Good fit for teaching segmental vascular anatomy in gross anatomy, back and abdominal wall modules, and neurosurgical or orthopedic spine courses that cover posterior element blood supply.
