Description
Anterior thoracic anatomy is rendered through a semi-transparent obese adult male torso, bringing the trachea into view as it descends in the midline and bifurcates at the carina into the right and left main bronchi. Each lung sits lateral to the mediastinum beneath the clavicles and behind the anterior rib cage, with the right lung divided into superior, middle, and inferior lobes and the left lung into superior and inferior lobes with the expected cardiac impression along its anteromedial border. Inferiorly, the diaphragmatic domes form the lung bases, with the costal margins framing the lower thorax. Obesity changes the mechanics that learners often struggle to visualize: increased chest wall mass and a more cephalad resting position of the diaphragm can reduce functional residual capacity and promote dependent atelectasis, particularly in the lower lobes. That makes this anterior relationship of lobar anatomy to the ribs, clavicles, and diaphragm practical when discussing perioperative ventilation, obstructive sleep apnea overlap, or why supine patients desaturate during sedation.
