Description
Anteriorly, the right and left lungs occupy the lateral thoracic cavities on either side of the mediastinum, with the trachea descending in the midline and bifurcating into the right and left main bronchi at the carina just superior to the pericardium. The right lung lies more vertical and broader, its deep oblique fissure running inferoanteriorly to separate superior and inferior lobes, while the left lung’s oblique fissure slopes down toward the costodiaphragmatic recess adjacent to the left dome of the diaphragm. Medial lung borders contour around the heart, and the lung bases rest directly on the diaphragm’s superior surface. Alveolar parenchyma fills the distal field. This anterior perspective makes fissural anatomy easy to teach, and it also aligns with how clinicians think about lobar localization on auscultation and radiography when pneumonia, aspiration, or atelectasis respects fissural boundaries. The close apposition of pericardium, heart, and central bronchi clarifies why left lower lobe collapse can silhouette the heart border differently than right middle lobe disease, and why diaphragmatic elevation (for example from phrenic nerve palsy) reduces basal lung volume. It is a practical map for bronchoscopy orientation and for anticipating pleural spread along the oblique fissures. Clear landmarks matter.
