Description
Seen from a male lateral perspective, the urinary bladder (vesica urinaria) is rendered with its detrusor surface and a superficial lymphatic plexus coursing across the lateral wall toward the vesical collecting vessels. Efferent lymphatics track posterolaterally from the bladder base and neck, running in the expected company of the superior and inferior vesical vascular pedicles, then converging toward pelvic nodal basins along the internal iliac and obturator regions. Inferior to the bladder neck, the prostate gland sits immediately caudal to the trigonal area, and the proximal urethra descends from the internal urethral orifice. Orientation is clear. Lymph channels remain predominantly lateral and posterior compared with the anterior dome. This lateral mapping matters because bladder lymphatic drainage is not uniform: lesions of the posterolateral wall and base often drain first to obturator and internal iliac nodes, while anterior and superior regions can reach external iliac pathways, shaping nodal staging and operative planning. For radical cystectomy or prostate-sparing approaches, surgeons use the vesical vascular pedicles as practical guides to where lymphatic collectors and sentinel nodes concentrate, and the obturator fossa becomes a common site of clinically relevant micrometastasis. It also clarifies why bladder cancer can present with pelvic nodal disease even when the primary tumor appears confined on cystoscopy.
