Description
Seen from an inferior perspective, the marginal sinus forms a circumferential venous channel along the internal margin of the foramen magnum, hugging the occipital bone at the craniovertebral junction. Posterior to the clivus and basilar part of the occipital bone, it lies adjacent to the medulla oblongata as it transitions into the upper cervical spinal cord. Lateral continuity with the dural venous system is suggested by its connections toward the sigmoid sinus region and by emissary pathways that communicate inferiorly with the vertebral venous plexus. Cranial nerves and basal brain landmarks provide orientation, including the brainstem, cerebellar hemispheres, and proximal segments of the lower cranial nerves as they course toward the jugular foramen and hypoglossal canal. For teaching and clinical correlation, the marginal sinus matters because it sits exactly where neurosurgeons and skull base surgeons work during foramen magnum decompression, far-lateral approaches, and occipitocervical instrumentation. Bleeding here can be brisk and obscuring, and its venous communications help explain why infection or thrombosis can spread between intracranial dural sinuses and the cervical venous system without a valvular barrier. A small structure. A frequent surprise. The inferior viewpoint clarifies how the sinus encircles the foramen magnum rather than running as a single midline conduit.
