Description
Centered on the anterior knee, the medial patellofemoral ligament (MPFL) spans from the superomedial margin of the patella to the medial femoral condyle region, near the adductor tubercle and medial epicondyle, running deep to the medial retinacular tissues. The patella sits anterior to the distal femur, with the patellar ligament coursing inferiorly from the patellar apex to the tibial tuberosity on the proximal tibia. Articular cartilage caps the trochlear surface of the femur and the patellar facets, while the joint capsule and medial collateral ligament lie medial to the joint line, reinforcing the femorotibial articulation. Anterior visualization of the MPFL matters because it is the primary passive restraint to lateral patellar translation in early knee flexion, before the patella is fully engaged within the femoral trochlear groove. Patellar dislocation, often in adolescents and young athletes, commonly tears the MPFL at its patellar attachment or at the femoral origin, and that injury pattern influences both MRI interpretation and operative planning. Small errors in femoral tunnel placement during MPFL reconstruction can overconstrain the patellofemoral joint and drive medial patellar overload. Alignment is unforgiving.
