Description
Cut through a left lateral section of the male skull, the hypoglossal nerve (CN XII) is followed to the hypoglossal canal in the occipital bone, where its meningeal branch (recurrent branch) turns proximally and runs posteriorly and superiorly back into the posterior cranial fossa. Dura mater lining the clivus and adjacent occipital squama sits medial to this fine nerve twig, while the jugular foramen and jugular bulb region lie posterolateral and the petrous temporal bone frames the inferior margin of the cranial cavity. Portions of the cerebrum are exposed superiorly, with pial vessels mapped across the cortical surface, and major intracranial vessels are color coded to clarify their course relative to the skull base foramina. Anatomists and skull base surgeons often overlook this recurrent meningeal branch because of its tiny caliber, yet it is one of the cleanest examples of a cranial nerve giving a dural branch through a bony canal, a pattern that mirrors meningeal contributions from the vagus and upper cervical nerves. This trajectory becomes clinically relevant when planning approaches around the hypoglossal canal and jugular foramen, where drilling, tumor resection (for example, hypoglossal canal schwannoma or jugular foramen lesions), or occipital condyle work can place small neural twigs and their accompanying dural innervation at risk. Seeing the branch re-enter the posterior cranial fossa helps explain how dural pain referral and postoperative occipital headaches can persist even when the extracranial hypoglossal trunk is preserved.
