Description
Arising from the internal carotid artery and coursing laterally into the lateral (Sylvian) sulcus, the middle cerebral artery (MCA) is shown from a superior perspective as it fans across the convexity of the cerebral hemispheres. Cortical branches track over the frontal, parietal, and superior temporal opercula, with vessels crossing the precentral and postcentral gyri on either side of the central sulcus. The interhemispheric fissure separates left and right hemispheres medially, while the MCA territory expands laterally toward the frontal and parietal lobes. Sulcal patterns provide the surface landmarks. A superior view clarifies the MCA’s dominance over the lateral cerebral cortex, the territory most often implicated in classic large-vessel ischemic stroke. Proximal M1 occlusion threatens the lenticulostriate perforators that supply the basal ganglia and posterior limb of the internal capsule, while more distal M2 and M3 branch occlusions map to cortical deficits such as contralateral face and arm weakness and aphasia in the dominant hemisphere. This is the vascular tree interventionalists target during mechanical thrombectomy, and the same anatomic relationships guide neurosurgical planning around the Sylvian fissure during aneurysm clipping or tumor approaches.
