Description
Running along the posterior aspect of the distal arm, the middle collateral artery (arteria collateralis media) branches from the profunda brachii artery and tracks inferiorly toward the lateral epicondyle of the humerus. Its course lies deep to the triceps brachii, crossing posterior to the humeral shaft and remaining lateral to the ulnar nerve as that nerve passes behind the medial epicondyle. Distally, the vessel approaches the lateral intermuscular septum and participates in the periarticular arterial network around the elbow. Small accompanying veins may be suggested, but the arterial pathway is the point. Posterior visualization of the middle collateral artery matters when you need to explain or anticipate elbow collateral circulation during interruption of the brachial artery, whether from supracondylar fracture, arterial line complication, or ligation. The anastomosis between the middle collateral artery and the recurrent interosseous artery helps maintain perfusion across the elbow when proximal flow is compromised. This relationship also becomes concrete in posterior approaches to the distal humerus, where deep dissection through triceps risks injuring branches of the profunda brachii and the radial nerve in the spiral groove.
