Description
Arising from the brachial artery proximal to the elbow, the inferior ulnar collateral artery tracks distally along the medial side of the distal arm and enters the posterior compartment to course behind the medial epicondyle of the humerus. From this posterior perspective, its relationship to the ulnar nerve in the cubital tunnel and to the medial head of triceps brachii is logically emphasized, with the vessel descending toward the olecranon and proximal ulna. Distally, the artery contributes to the periarticular anastomosis by joining branches of the ulnar artery, most commonly via the posterior ulnar recurrent artery, across the posterior aspect of the elbow. Elbow perfusion depends on these collateral pathways when the brachial artery is temporarily compromised by flexion, edema, or surgical control. This is the artery you want in mind during posterior-medial approaches to the elbow, including triceps-splitting exposure and procedures around the medial epicondyle, because injury can reduce collateral flow and increase hematoma risk in a tight fascial space. Small vessel. Big consequences. Teaching value is strongest in sessions on upper limb vasculature, periarticular anastomoses, and the functional anatomy of the cubital tunnel, where students often confuse the inferior ulnar collateral artery with the superior ulnar collateral and with the radial collateral system laterally. It also suits surgical atlases and clinical education materials addressing elbow trauma, ulnar nerve decompression, and vascular repair planning around the distal brachial artery.
