Description
Running longitudinally on either side of the thoracic spinous processes, the longissimus thoracis is highlighted within the erector spinae mass, positioned lateral to spinalis thoracis and medial to iliocostalis thoracis. From a posterior view beneath the skin, the muscle belly occupies the paraspinal gutter, with segmental attachments spanning the thoracic transverse processes and inserting onto adjacent ribs. Superficial to it lie the broad sheets of trapezius superiorly and latissimus dorsi inferolaterally, while the deltoid caps the shoulder and the gluteal musculature frames the inferior field. Clear layering. This perspective matters because the longissimus thoracis forms a major component of thoracic extension and ipsilateral lateral flexion, and it is routinely implicated in paraspinal strain patterns that present as unilateral pain adjacent to the thoracic spine. Palpation, dry needling, and EMG targeting all depend on appreciating its relationship to the spinous processes medially and the rib angles laterally, where thoracic dorsal rami traverse and where symptom referral can be mistaken for costovertebral or facet-mediated pain. For posterior spine exposure, it also helps to distinguish what is superficial (trapezius, latissimus) from what must be split or retracted to reach deeper elements of the posterior thoracic wall. Ideal applications include anatomy and kinesiology teaching on the erector spinae, sports medicine texts discussing posterior chain injuries, and surgical education materials for thoracic or thoracolumbar posterior approaches where paraspinal muscle handling influences postoperative pain and function.
