Description
Centered within an anterior torso of an obese adult Black male, the esophagus descends in the midline through the diaphragm to the stomach in the left upper quadrant, with the greater curvature oriented inferiorly and laterally. Superior and to the right, the liver occupies the right hypochondrium and epigastrium, its inferior surface closely related to the gallbladder nestled along the right costal margin. Inferior to the stomach, jejunal and ileal loops fill the central abdomen, framed peripherally by the large intestine, with the ascending colon on the right, transverse colon crossing superiorly, and descending and sigmoid colon tracking into the pelvis toward the rectum and anal canal. Visceral topography is the point. Obesity changes what clinicians expect to palpate, resect, and image, so this layout supports teaching that organ position is consistent even when the abdominal wall and mesentery carry increased adipose tissue. Bariatric and upper GI procedures, including laparoscopic sleeve gastrectomy or Roux-en-Y gastric bypass, hinge on reliable orientation to the gastroesophageal junction, left lobe of the liver (often enlarged in metabolic-associated steatotic liver disease), and the transverse colon and small-bowel mesentery that can obscure the operative field and increase internal hernia risk at mesenteric defects. Radiology trainees also use this arrangement to localize pain patterns and interpret CT findings in obese patients where acoustic windows and physical exam are limited.
