Posteriorly oriented anatomy places the esophagus descending through the mediastinum to the stomach, which lies in the left upper quadrant with its greater curvature directed inferiorly and laterally. Superior and to the right, the liver occupies the right hypochondrium, with the gallbladder tucked on its visceral surface; the pancreas sits retroperitoneally, running transversely behind the stomach toward the splenic hilum. Centrally, the jejunum and ileum form tightly packed loops suspended by the mesentery, while the large intestine frames them, from the cecum in the right lower quadrant up the ascending colon, across the transverse colon, and down the descending and sigmoid colon to the rectum. Fat planes are prominent. In an obese male habitus, this posterior “through-the-back” perspective clarifies which segments are intraperitoneal versus secondarily retroperitoneal, a distinction that drives both pain patterns and operative planning.
