Description
Anterior full-body standing view of an obese adult Black male with semi-transparent integument reveals the alimentary tract from the cervical region through the abdominopelvic cavity. The esophagus descends in the midline posterior to the trachea, traverses the diaphragm, and joins the stomach in the left upper quadrant, where the greater curvature lies inferior and lateral to the lesser curvature. Superior and to the right, the liver occupies the right hypochondrium and epigastrium, partially overlapping the stomach and proximal duodenum, while centrally the small intestine (jejunum and ileum) forms coiled loops that are framed peripherally by the large intestine, with the ascending colon on the right, transverse colon superiorly, descending colon on the left, and sigmoid colon coursing inferomedially toward the pelvis. Visceral layering is apparent. Depicting the digestive system in the context of obesity matters because increased subcutaneous and visceral adipose tissue changes surface landmarks and operative working space, altering how clinicians conceptualize organ depth, mesenteric fat, and access to the stomach, liver, and colon during laparoscopy. That spatial problem shows up in bariatric procedures (sleeve gastrectomy, Roux-en-Y gastric bypass), where the left lobe of the liver may obscure the gastroesophageal junction and the greater omentum and thick mesentery can limit visualization and retraction.
