Description
Across the dorsum of the male foot, the dorsal metatarsal veins course in the intermetatarsal spaces over the shafts of the first through fifth metatarsals, receiving dorsal digital veins from the toes and converging proximally toward the dorsal venous arch. Superficially, these veins lie anterior to the metatarsals and dorsal interossei, deep to the skin and superficial fascia, and they run in parallel with the dorsal metatarsal arteries arising from the dorsalis pedis artery. Proximally at the level of the tarsus, the superficial network continues medially toward the great saphenous vein via the medial marginal vein and laterally toward the small saphenous vein via the lateral marginal vein, with extensor tendons (extensor hallucis longus and extensor digitorum longus) crossing them in the anterior compartment of the foot. Venous anatomy on the dorsal foot matters because it is the commonest access point for peripheral venous cannulation when upper-limb sites are limited, and the caliber and course of the dorsal metatarsal veins influence both success and complication rates. Small branches are easily injured during dorsal midfoot approaches (for example, dorsal plating of metatarsal fractures or incision over the Lisfranc complex), and postoperative edema can obscure the dorsal venous arch while increasing the risk of superficial thrombophlebitis. This is also the terrain where dorsal foot edema in heart failure or nephrotic syndrome becomes clinically obvious. Surface anatomy you can palpate and see.
