Description
Across the dorsum of the male foot, the dorsal metatarsal veins run longitudinally over the metatarsals, beginning distally as dorsal digital veins and converging proximally toward the dorsal venous arch. From there, tributaries course toward the anteromedial ankle, where the great saphenous vein typically ascends anterior to the medial malleolus, and toward the anterolateral ankle, where the small saphenous vein commonly tracks posterior to the lateral malleolus after draining the lateral end of the arch. Superficial venous channels lie immediately deep to the dermis and superficial fascia, making their linear paths and interconnections apparent against the bony contour of the forefoot. Landmarks read cleanly. For teaching and clinical planning, the dorsal network matters because it represents the most accessible superficial venous bed of the foot and provides a predictable map of distal-to-proximal drainage into the saphenous systems. Venipuncture on the dorsum of the foot is still performed when upper-extremity access is limited, and the risk profile changes in patients with diabetes, peripheral arterial disease, or lower-limb edema, where dorsal veins may be fragile, thrombosed, or difficult to cannulate. The course across the metatarsals also helps explain why superficial thrombophlebitis or post-traumatic swelling can present as a tender linear cord along the dorsal forefoot. Use this dorsal perspective in gross anatomy and surface anatomy courses to correlate palpable bony landmarks with superficial venous patterns, and in nursing, paramedic, and phlebotomy training materials focused on lower-limb vascular access. It also fits well in patient-education handouts and clinical slide decks discussing saphenous vein drainage, superficial venous thrombosis, and site selection for peripheral IV placement in the foot.
