Description
Oriented from an oblique posterolateral angle, the pelvis and proximal femora are shown with the hip joint capsule opened to highlight the ischiofemoral ligament on a male specimen. Fibers arise from the ischial portion of the acetabular rim and adjacent ischium, then spiral superolaterally across the posterior capsule to blend with the zona orbicularis and the anterior aspect of the femoral neck near the greater trochanter. The acetabulum, femoral head, and neck sit lateral to the pelvic midline, while the lumbar vertebrae and sacrum remain superior and posterior as bony landmarks anchoring orientation. Capsular geometry is the story here. That spiral fiber direction explains function better than any text: as the hip moves into extension, the ischiofemoral ligament winds tighter and limits hyperextension and excessive external rotation, stabilizing the femoral head within the acetabulum. Clinically, this is the capsular structure most relevant to posterior capsulotomy and repair in hip arthroplasty, where inadequate restoration can contribute to posterior instability and dislocation risk.
