Description
Seen from the lateral aspect, the iliofemoral ligament (Y ligament of Bigelow) fans from the anterior inferior iliac spine and adjacent acetabular rim to the intertrochanteric line of the proximal femur, blending with the fibrous capsule of the hip joint. The femoral head sits deep within the acetabulum, and the ligament lies anterior to the head and neck while the greater trochanter remains lateral as a clear bony reference. Portions of the pelvis, including the iliac crest, acetabulum, and pubic segment, anchor the scene, with gluteus medius positioned superior and lateral to the joint over the outer ilium. Lumbosacral elements appear posterior and superior as proximal context for pelvic orientation. Functionally, this lateral view clarifies why the iliofemoral ligament tightens in hip extension and external rotation, limiting anterior translation of the femoral head and resisting hyperextension during upright stance. It is the main passive restraint that lets a person stand with less continuous iliopsoas activation. Small details matter. Appreciating the ligament’s course along the anterior capsule helps when teaching the difference between anterior and posterior instability patterns, and it frames why anterior capsulotomy or capsular laxity after arthroscopy can predispose to iatrogenic hip microinstability.
