Description
Posterior thoracic and upper lumbar anatomy centers on the longissimus thoracis as the intermediate column of the erector spinae, positioned lateral to spinalis thoracis and medial to iliocostalis. Segmental muscle bundles track longitudinally along the angles of the ribs and the transverse processes of the thoracic vertebrae, with deeper fascicles blending into the thoracolumbar fascia inferiorly. The spinous processes form the midline landmark, while the ribs arc laterally from the costotransverse region and partial scapular borders sit superficially over the posterolateral rib cage. Longissimus thoracis matters because its line of pull explains both extension and ipsilateral lateral flexion of the thoracic spine, and its firm attachments near costotransverse joints make it a frequent pain generator in mechanical thoracic back pain and costovertebral dysfunction. In posterior approaches for thoracic medial branch blocks, radiofrequency ablation, or placement of an erector spinae plane block, the relationship between the paraspinal muscle mass and the bony landmarks (spinous processes, transverse processes, and rib angles) guides needle trajectory and helps avoid unintended pleural violation. For teaching, the segmented fiber appearance also helps learners distinguish the erector spinae columns from deeper transversospinalis muscles such as multifidus, which lies closer to the laminae and spinous processes.
