Description
Posterior neck and upper thoracic anatomy are arranged to expose the deep semispinalis capitis in a male, with the occipital bone and external occipital protuberance superiorly and the cervical spinous processes aligned inferiorly into the upper thoracic spine. Semispinalis capitis lies medial and deep to the splenius capitis and trapezius, spanning from the transverse processes of the upper thoracic vertebrae and lower cervical vertebrae to the area between the superior and inferior nuchal lines of the occiput. Inferiorly, semispinalis cervicis and semispinalis thoracis sit in continuity with it, tracking along the posterior elements of the vertebral column near the zygapophysial joints. A deep extensor mass. Clear separation of semispinalis capitis from splenius capitis matters when teaching layered posterior cervical musculature and when interpreting pain generators in the upper neck. This muscle group is implicated in cervicogenic headache patterns and in extension or rotation pain arising from adjacent C2 to C3 facet joints and the greater occipital nerve as it emerges near the inferior oblique capitis and pierces more superficial musculature. For posterior cervical approaches, understanding which fibers run to the occiput versus to cervical spinous processes helps predict how retraction and subperiosteal elevation can affect postoperative neck stiffness.
